This seems to be one of the active Pi 4B threads about heating so throwing in my 2c (use case).
Running Kodi 18.7 for years, pretty much firmware updates only.
Can't say what the temperature was originally (maybe 50C), but nothing out of the ordinary even under load. Now, 3-4 years later I noticed the external ssd is running extremely hot these days. But things worked so no worries.
Then I was on a trip, forgot the remote and had to use a mouse, hence ended up using the 4th USB slot. Kinda guessing that was passing out some of the heat.
From now on, again guessing, maybe something got overheated too much inside. Cannot find anything lose or out of shape though.
All usb connectors are now "burning up" (not nice to touch at all) even if not in use.
Idling temperatures are now 61C, way over 70C with even minimal load.
SD card stopped working (read: cannot be re-formatted, gone), guess the heat killed it.
Well, at least I got my setup upgraded to Omega Image may be NSFW.

Drilled a few holes into the original plastic casing hoping that will help enough.
Going for a bigger solution media center in the summer so skipping upgrading to a Raspberry 5 and aluminium passive case.
Not too happy about design being a bit more flimsy these days. The original raspberry worked fine for, dunno, 10 years until Kodi 18.7 was too much for it CPU-wise, stutter.
4B has a RJ45 that never clicked properly but was too lazy to return it for such a minor fault. Maybe some other components were sub par as well then.
